This tutorial is an introductory guide towards meeting the requirements for recording, tracking and evaluating clients' progress towards achieving the outcomes of a HIV/AIDS supportive housing program. Emphasis will be placed on illustrating SMART goal setting using the Acuity Index (AI), service plans and progress notes systematically over a specific timeframe.
